Lean Six Sigma Questionnaire

I'm currently researching a PhD and for my thesis I’m analyzing the key differences between different industry sectors and countries implementing Lean Six Sigma, and I’m hoping to publish an academic paper on the results.

I'm also researching environmental drivers for using lean six sigma and how this is affecting stakeholders within organisations

I have created a questionnaire for users of Lean and/or Six Sigma. It takes 5-10 minutes to complete and it is completely anonymous. Please pass to colleagues or friends wherever possible.
